Add Narrative Description extension to items

The IG summary tool chooses different descriptions for items. This is partly because the item descriptions in the narrative are different from those in the structure definition (like in US Core). MADFSH could add an extension so that they are present there as well and could be used by tools like the[ ig-summary tool ](https://github.com/mitre/ig-summary)
