add sample apps with sample output to docs

we can pull some code and results out of the drmem test suite.

I should also add the examples from my Tech Talk slides to the docs

- added a simple example of an unaddressable error with a diagram. I'd
like to add more examples and more diagrams, including from my Tech Talk:
this is just a simple step in that direction.
- mentioned std::string and heuristic disabling runtime options for leaks
