Quick create contact modal: more fields

Description
The modal doesn't match the create contact page. The create contact would be better if the other fields were available to be completed on first creation, and not have to create then edit (add more info) later.
Both the page and the modal could be enhanced by adding other fields:
- phone
- source
- status
- initial comment
