Custom Module Action Menus - Extension Point

## Description of documentation change
In DNN 9.4 we now have the ability to customize the module action menus on a per install basis. I'd like to add documentation surrounding this new extension point and an example of how they can be extended.

Consider this issue a placeholder until I can round up my documentation.
