Allow reporting inactive mentors / mentees

**Is your feature request related to a problem? Please describe.**
Currently member don't have the option to notify us about inactive mentors / mentees so they approaching via Github issues, chat, Slack (Or don't tell us at all which is the worst).

**Describe the solution you'd like**
Add a button / link to make it easier to report

**Additional context**
It's good to know a bit background.
We can show a modal with some questions
- Did the person ever communicate?
- How long since the last communication?
