User Story - As a volunteer / service user I want to be able to sign up & add my details directly to the dashboard, so my details can be automatically added into the system without having to go through MIN staff

- [ ] Sign up page / website is hosted & available 24/7 so users can enter details when convenient
- [ ] User can add information in each of the boxed in the same fashion
- [ ] Default value in status column should be “new” for any profile added directly via the sign-up page
- [ ] Errors with descriptions & advice on how to resolve are shown when info entered incorrectly
- [ ] Confirmation screen shown when details added successfully
- [ ] Details entered successfully are entered into the back end DB & held securely
