Proof of Concept: generate "scaffold" screening form purely from eligibility checks

Given a Benefit with Eligibility Checks
When I visit the Form Editor
Then I should be able to start with a generic form that asks Yes/No questions about all the eligibility checks

This is to give the Benefit Analyst a starting place for creating a screening form.
