Add a progress bar to screening form to help users see where they are

Right now, users don’t have a clear way to track their progress as they move through the screener. This can make the experience feel confusing or incomplete. Let’s add a simple progress bar or step tracker at the top of each page so users know how far along they are and what’s left.
