[Bug] Cannot tell if feedback form has been submitted or not

Description
Steps to reproduce:
1. Go to [Feedback form](https://balancertestsite.com/feedback)
2. Fill in all fields except Feedback type
3. Submit the form
4. Get error in the above screenshot
5. Go back and select a Feedback type
6. Hit submit again
7. Expected results: Error message is gone, success message appears, form fields clear
8. Actual results: Error message persists, no success message, I cannot tell if the form has been submitted
