Diet management - Forms discussion

mainly we didn't have a completed specification right now. so a lot of things has a blank page.

But the main problem that we should solve from a design perspective: create a form, where we asking a user for filling personal data. it's a usual way to collect data from patients. But forms is not cool. So we need to find a way how to get a data from user and onboard with the process, of what we'll do this data. i think this will add some interest and give more information to user.
