Display version numbers in output

In the generated XLSX files (query and annotate), adopt a line at the top that displays information about which versions were used:

- [ ] SASTA version
- [ ] Method category + version (`method.name`)

Don't include versioning in the forms for now, this will have to be done in consultation with Jan Odijk.

Locate the XLSX generation entry points for the query and annotate outputs, then trace how the workbook's top rows are assembled. Add a top line showing the SASTA version and the method category plus version from method.name, while leaving forms unchanged. Done means both generated XLSX paths display the requested information.
