Map Web Application: Site doesn't load and returns error

I've set up the map web application according to the steps in the software manual. However, when I go to load the site it returns the error indicated in the attached picture. This error was discussed at the CUAHSI conference in June regarding the server
at Idaho State University.

Review the software manual's setup steps and inspect the attached error image first, then reproduce the map web application failure on the server at Idaho State University if access is available. Identify the specific loading error and its server-side cause; done means the site loads successfully without that error.
