The form should display for which catalogue item it is for

The form should display for which catalogue item it is for (or workflow) because the same form can be reused for many catalogue items and it's unclear why it is repeated in the application.

We can show the catalogue item name in the form as the first information. We can also consider showing the organization it is for and the localized form name. Perhaps for the handler/reviewer we should also show the organization it is for.

For applications with only one form, don't show it.
