Create Registration Page

**Problem Description**
users of this app can't be identified. There is a need for users to easily identify and introduce themselves to the app via a friendly registration page.

**Solution Description**
we have various users of our app and we would want to identify them and show them various form fields depending on the options they choose during the registration process.

Attatched is a file that will aid in taking the user through the registration process.
