UI for defining a scoring function via formula

Scoring functions are frequently calculated on the basis of more than one measurement. It would be desirable to have a window where a user can define the scoring function as a formula. Building on this, it would also be benefitial to have the front-end remember that a formula was used, and so change the input fields for data so the user simple enters each measurement value, after which the score is automatically calculated.
