101-app-service-certificate-standard problem

When I run this template the deployment hangs on the `Microsoft.CertificateRegistration/certificateOrders/certificates` resource. It will run for hours but never get past that resource. Any ideas what might be wrong?

FWIW I have granted the RPs access to Key Vault, as per the prerequisites in the README. Also, I am setting the `rootHostName` to my Traffic Manager URL, which is what I use if I do this manually in the Azure portal by clicking on the "Add App Service Managed Certificate" button within the "TLS/SSL Settings" tab. When I add the certificate in the portal it takes a few seconds.

Start with the 101-app-service-certificate-standard template and its README prerequisites, then reproduce the deployment using a Traffic Manager URL as rootHostName. Inspect the Microsoft.CertificateRegistration/certificateOrders/certificates resource while it runs; done means the template deployment completes instead of hanging there.
