Azure Functions templates should include Application Insights

Since we recommend Application Insights integration by default, it should be added to these two ARM templates. (@mattchenderson)

Start with 101-function-app-create-dynamic/azuredeploy.json and 101-function-app-create-dedicated/azuredeploy.json, then review the linked Azure Functions ARM topic for context. Add the recommended Application Insights integration to both templates and verify that each template reflects the requested default integration.
