Validation errors in Azure Template "sqlvm-provisioning-csp"

Hi,

I am getting error while deploying
https://github.com/Azure/azure-quickstart-templates/tree/master/sqlvm-provisioning-csp
1. When clicked on Deploy to Azure
2. I filled all the required parameters in Azure
3. Once press Purchase giving error "There were validation errors. Click here to view details."

Please suggest what i am missing ?
