Telemetry: some data are missed in new modules

## Description

The subscription-id is not reported
The module name is “Az.{module}.private”
The commandinvocationname is off
The powershellversion does not seems right

Reproduce the telemetry issue with the latest released Az package, checking a new Az module and the fields listed in the report. Compare the subscription ID, module name, command invocation name, and PowerShell version with the expected values; consult documentation/troubleshoot-module-load.md if module importing is involved. Done means all four telemetry values are reported correctly.
