[telemetry] New azd data dashboard, separate from the template dashboard

## Problem

One dashboard is serving two very different audiences:

- **Template authors** want per-template health — is my template working, where is it failing.
- **The azd team** wants product data — who is using azd, on what surface, with what success.

Current dashboard is very template driven and we need to create a new azd data dashboard for case 2.

## What we'll do

- Keep the existing dashboard as the **template dashboard**, for template authors.
- Build a **new azd data dashboard** for azd product data, with its own ACA links.

## Done when

- Two dashboards exist, each with a clearly stated audience.
- The azd data dashboard covers users, surfaces, cohorts, and feature usage.
