[Knowledge] Add file interaction: File name input field

### Description:
The file name input doesn't have a required astrisks. However, the "Add" primary button is disabled.

### Question:
Is creating a new name for the file a requirement? I would expect this to be optional. Also, it's odd to have them create a name, but not have functionality to update the name and description after upload.

### Recommendation:
If it is required:
- Add a red "Asterisks" next to the column label: Name
- Change placeholder text to, "Create a name for this file" -- Esther Fan to approve and or provide final copy
