[Custom templates] Profile tab: By, Contact and Summary fields

### Description
- `By` field: This should be a dropdown so that we are using a controlled vocabulary for the By field. I recall we were going to use the tenant for this value. This will reduce issues with inconsistencies due to typos and other human errors. character length in the Template Card and Detail page.
- `Contact` field: Add this field. This is a text field that will allow users to add a person or team as a contact. Limit character length.
- `Summary` field: Add character limit since so that it doesn't overflow in the detail side panel.
