Visual Studio crashes on Add New Azure Function dialog

Description
I've already set the appropriate environment variables and updated visual studio, however:
1. When creating new solutions, 3.0 is still not an option.
2. From within a solution that I manually upgraded to 3.0, when I try to add a new Azure Function, VS crashes.
