Description

Hi Azure-Samples team,

I’m a Microsoft FTE (htinoco@microsoft.com), submitting from my personal GitHub account because Enterprise Managed User restrictions prevent my Microsoft-managed GitHub org from hosting public repositories.

The publishing guidelines currently point to https://forms.office.com/r/cy1ACkEGK5, but that form returns 404.

I would like to submit this azd template for review/publication:
