Jetpack Subscription form add `I agree to T&C` check-box

Add a `consent/ I Agree` checkbox to the Jetpack subscriptions form with a link to the Terms and Services page like we do for the GDPR consent banner.

> I just got some fresh information from the responsible department of the Swedish government saying that a checkbox is actually necessary, as well as link to a page explaining the subscribers’ rights in the light of GDPR. Therefore I suppose that Jetpack will need to add these “features” to the subscription widget, at least if your intention is to continue having users/customers in Europe.
