Implement support for anonymous Apple ID

The present version v1.3.1 shows the option for anonymous linking, to pass Apple review. However, it is unsupported.
The proposed solution:
- [ ] Accept the ID (email or anonymous)
If the record is not found
- [ ] "Your coach record will need to be verified by the Scores Team. Please provide the following additional information
- Full name
- Phone number where we can reach you
- email
- Date of Birth
- [ ] The system creates a _provisional_ Contact record with this information
- [ ] The system alerts the Scores Staff to review and merge the contact with a prior record or make it a full Coach record
Other followup by Scores Staff will be manual
- [ ] At the end of this, a message "Thank you. We will contact you when your account is verified."
- [ ] Show a link to ScoresU.org
