(bug) Assessment Create not active in Production (App or API?)

Description
To reproduce:
1. Select a session for an active team with enrollments
2. select the Create Assessment action on the student attendance record (icon right of the student name on the Attendance View)
3. Select type "Activity Feedback" in the modal
4. Enter a short comment in the text field e.g. "I feel good after soccer practice" and Submit
Result:
App hangs with server-connect animation running forever.
No log entries on the API salesforce-data-api-prod for an Assessment are appearing suggesting a config issue on the App.
