Enhancement for support of multiple public identities

For call originations it would be useful to be able to select the PUID to be
used, if multiples have been passed to the client in the P-Associated-URI
header(s).

For call termination, in addition to showing calling party information, it
would be useful to show the called party line, again, if multiples PUIDs have
been passed to the client.
