Add google form input UI to index.html

Currently the only way to run tttc on a dataset pulled from google sheets is to clone the repo, run the dev server, and then run [the googlesheet script](https://github.com/AIObjectives/tttc-light-js/blob/main/examples/googlesheet-with-config.mjs).

We should augment the current [index.html](https://github.com/AIObjectives/tttc-light-js/blob/main/public/index.html) report creation UI to take all the necessary fields for using a google sheet, instead of a CSV upload. See the CONFIG object in the googlesheet script above for the set of fields necessary for google sheet input.
