Improve confirm page to display more useful information

The confirm page often shows insufficient information about the request being made through the adapter.

* Some views are empty, aside from the title and the requesting domain (assets)
* As seen in the screenshot below, custom transactions show no details about the transaction (ie. input and output notes) beyond the sender and recipient
* The look and feel of the confirm page should be uniform across all views
